The Screenings Request Log template is designed to support a filmmaker in facilitating community screenings of their film. 

Film impact campaigns often integrate some kind of screenings initiative. This template can help track screening requests, screening host information, licensing fee payments, and other screening details. The information you collect through a form like this can also be used later on for reporting purposes. While this template is based on Peace is Loud’s previous community screenings initiatives, you can modify the spreadsheet to best suit your campaign’s needs.
